Osteoporosis may
be
primary
(due
to
aging), secondary
(due
to
disease or medications) or sporadic
(in
females 8-14, years usually with family
history). Most cases are
primary. The
trabeculae (tiny
bridges
forming
the
network of
the
cancellous
bones)
thinning
weakens bones,
e.9.,
the
vertebrae of the spine, leading to com-
pression fractures; other sites
are
the
femur (thigh bone). Osteoporosis
is
closely associated with
the
declining
female
hormones
at
menopause,
although another
(less
common) type
of osteoporosis also occurs at an older
age (>70)
in
both
men
and women. The
world's average incidence
of
osteoporo-
sis is around
10o/o;lot
China, around
67o.
Menopause,
lack
ol exercises, poor
calcium absorption and even
smoking
and
excessive drinking
are
relevant.
Remodeling
of bone
is
on
the use-more/
stronger principle. The US
National
lnstitute
of
Health (NlH) recommends
for menopause
1.5 g
of calciumlday,
e.
9., 500
ml
of milk;
as
the Chinese aver-
age
diet
lacks adequate
calcium,
supplements
(with
vitamin
D
400 iu/day)
can
thus
be
iustilied;
regular muscle
exercises, including posture correcting
and purposeful ones, especially for the
spinal column; enhancing balance and
coordination practices
help
to
prevent
falls.
With the
very
high
chance
of
asep-
tic
(not due to infection) necrosis
ot
the
femoral neck,
replacement by artificial
hip
is
worthwhile.
