FOR
STROKE
PATIENTS
lnterventional radiology
is
a
"minimally invasive" procedure
in
which
the
insertion
of
tubes into
blood
vessels
is
done
under
close
radiology
or X-ray scrutiny, e.9., digital subtrac-
tion
angiography (DSA),
to
achieve
therapeutic objectives at the site of the
lesion, e.9., tunneling inside
a
vessel
to dissolve
a
thrombus
or clot causing
a stroke.
Such
"thrombolylic"
proce-
dures are getting more and
more
available
lor
managing acute stroke
provided
that
it can be given
within
hours after
symptom
onset
of
the
attack.
Serious side effects, e.9., symptomatic
intracranial hemorrhage (bleeding
in
the
brain) may also
occur.
ln fact,
the
best
results are those
in
whom thromboly-
sis can be
applied
within 3
hours;
administration at 3-6 hours produces
less satisfactory.
Newer
agents
are
developed to widen the time
window.
Thus,
it is
impofiant
to
assess
the
stroke
patient
very
promptly
whether there
is
presence
or
absence
of
a
vessel
occlusion. The salvageable tissues are
those lacking blood supply
(for
the time
being) but
nevertheless are still
viable
must
be
distinguished from the irrevers-
ibly damaged completely dead tissue
core that
would
never benefit
lrom
the
thromoblytic procedure. As
newer,
safer
and more
effective
agents
for the
throm-
bolytic procedure are now
being tried,
the
prospects
for
stroke
is
becoming
befter with time.
